Service to the world!

Hello everyone!
Nice to have you here on my spot!  I welcome you all to my idea of helping the world be a better place to live in.

How do i do that?  Well,  let me just give you a quick background of myself.

I have been a medical doctor for almost 10 years. I have worked both in government and private hospitals.  I have seen my share of the most morbid (horrible medical) cases and also the most easy ones.  I have encountered the most polite patients to the most arrogant ones, including the drunkards with matching super alcohol breath. (That's one of the many reasons why some doctors wear a mask.)
Being a doctor wasn't really my passion,  I was forced by my mother to take this course (i'm sorry Ma for sharing this online).  My real passion is to help people.  That is why i have endured the 4 year proper medicine course (mind you, i already had a 4-year bachelor's degree before entering med school.)  Those sleepless nights,  the microscope,  the cadavers and the big books... I had conquered them all so that when I graduate, I can start helping other people.

Now,  getting back to my question, how can i help the world?  I will give online medical advice to all of you!  Isn't that awesome,  well,  it is for me.  It will make me happy just to help another person with his/her medical problem. This is totally free,  no fees! (I just love how it rhymes.)

Send in your questions my friends.  I will answer you to the best of my abilities.

    1. Hi Alpha RuXaRo, I will cover that very interesting topic in my upcoming blogs. My first advise is not to take any slimming or diet pills yet. You should exhaust other means to lose weight first. More answers to come, will inform you once the blog is done.
Thanks for visiting my spot. Have a nice day!

    2. Life is what you make of it. You could be the wise man, the fool, the poor or the rich guy. The decision is entirely yours. I say, follow your bliss.

      Thanks for sharing your thoughts.

  3. Well, the discussion of the meaning of life is way beyond a blog. Different people have different answers depending primarily on their experiences. Let me gather my wits and give you a worthwhile read, Alexis. Have a safe journey and think positive thoughts always.

    1. Hi there!
Yes, certain foods can make your farts smell really bad. Cabbage, onion, garlic, broccoli and collard greens which are high in sulfur really causes horse shit flatulence.
Flatulence (farts) is really foul smelling even if you aren't a vegetarian. Flatulence is released when the bacteria in our large intestine break down unabsorbable carbohydrates. Your friend, being a vegetarian, consumes a lot of those stuff.

  5. Doc, have you ever encountered a case of congenital brain tumor about half the size of the brain of the neonate? He presented with hydrocephalus at birth, detected by utz otherwise it looks unnoticeable. Requested MRI revealed this tumor. The only procedure this baby can sustain for his age is shunt insertion. My desperate question is the chances of survival. A sample of the tumor will be taken for biopsy during shunt procedure. Just wanna hear of any similar case... and how the baby is doing.

  6. Hi there!
Fortunately or unfortunately, i haven't encountered a neonate with a confirmed case of congenital brain tumor. Although, i have seen many babies with very large heads due to hydrocephalus, some of these babies, for sure, has an underlying tumor. I'm sorry to say that the chances of survival (prognosis) is very poor. This is because of the size of the tumor and that the body of the newborn seldom survives a major operation. However, there are small brain tumors, if detected early, can be surgically resected. Furthermore, the kind of tumor is also a factor in surgical success, for example, choroid plexus tumor. The rapidly fatal ones are the teratomas. I would like to know the biopsy result of the tumor, please email me at questions.medical@gmail.com

  7. Doc, what is the safest and quickest way to quit smoking? aside from a strong will which clearly I do not have.. hehe. is it safe to go cold turkey right away or gradually minimize it? thanks a lot doc. =)

  8. Hey Cyril, what's up? If you don't to use your will, then you can use the 'force' hehehe. Kidding aside, if you want to quit smoking, you should do it gradually. You will have severe withdrawal symptoms if you stop abruptly. Headache, nausea, diarrhea/constipation, irritability, anxiety are some of the symptoms. Over the counter medicines to help with quitting include nicotine gum, nicotine patch and nicotine lozenge. These drugs deliver low level of nicotine to your body to lessen the withdrawal symptoms and ultimately help you with the process of smoke cessation.

  9. Dr A , just want to ask your opinion regarding the surgical management of bilateral undescended testes (cryptochordism) as to when is the right time for surgical intervention in relation to the risk of infertility and carcinoma. Ty

  10. Hi Melvin, you have quite an interesting question.

The appropriate timing would be one year old or younger to decrease the risk of infertility and carcinoma in babies with bilateral undescended testes. Surgical management can start as early as 6 months old.

    Many factors are considered.

If the testes are palpable then the decision would be straightforward. Let us give time for the testes to descend until 6 months, surgical correction can be done if still undescended.

For bilateral unpalpable testes, the first thing to do is to confirm the presence of testicular tissue through karyotyping and hormonal work up. If present, laparoscopic abdominal exploration should be done, which is the gold standard. The latest laparoscopic instruments with a diameter of 3mm can be safely used in boys one year or younger. If gonadal vessels are viable then correction of the abnormality can be done on one testis and the other testis will be operated 6-12 months after.

Lastly, boys with bilateral cryptorchidism are associated with increased chances of other organ anomalies, one example is congenital adrenal hyperplasia. Referral to a pediatric endocrinologist should be done to optimize the baby's health in preparation to the surgical procedure. Health optimization must be the main consideration when to schedule the operation because as we have all know, babies are high risk for any major surgery.
